About The Position

This position is located at the Behavioral Health Hospital with occasional float to Alamance Regional Medical Center. The role involves disposition planning, addressing Social Determinants of Health (SDOH), and managing cases related to domestic violence, substance abuse, and homelessness. This specific role will focus on SNF placement and home health/DME arrangements for adult Neurology patients, primarily those with Stroke and Seizure conditions. Opportunities to assist with other patient populations may arise, but the core job functions remain consistent.

Requirements

• Minimum 1 year of experience required
• BLS required
• NC license required
• EPIC experience

Nice To Haves

• Previous experience as an LCSW in a psychiatric or behavioral health setting
