About The Position

This is a 13-week contract position for a Licensed Clinical Social Worker (LCSW) at the Heart Transplant Program in Portland. The role primarily involves providing outpatient support for advanced heart failure treatment evaluations, including heart transplant and mechanical heart assessments. There may also be some inpatient evaluation requirements. The transplant clinic serves approximately 250 total transplant patients and 40 VAD patients, with an average of 15-20 transplants performed annually. The ideal candidate will have experience with transplant and/or heart patients and be comfortable collaborating with an inter-disciplinary team.

Requirements

• OR LCSW certification
• BLS certification
• 1 year of experience
• Comfortable interacting with an inter-disciplinary team

Responsibilities
