About The Position

Valor Healthcare is seeking a part-time Licensed Clinical Social Worker (LCSW) specializing in Primary Care/Mental Health Integration (PCHMI) for their Community Based Outpatient Clinic (CBOC) in Columbia, TN. This role focuses on providing brief, solution-focused interventions for veterans within the primary care setting, following an evidence-based model of collaborative care. The PCHMI model aims to increase access to mental health services, assessments, and treatments. Clinicians will receive training and certification through the VA. Sessions are typically short, with a limited number per issue per year. The position offers a competitive salary, excellent benefits, generous time off, and a weekday schedule with flexibility within clinic hours (Monday-Friday, 8 AM - 4:30 PM). The role requires 16 hours per week.

Requirements

  • Must have a Master's degree, or higher, in Social Work from a program registered by the Department of Education, with specific training dealing with adult patients. Equivalent degrees (Counseling, Marriage and Family Therapy, etc.) will be considered and are subject to final approval from the local VA.
  • Must comply with and maintain all requirements for a valid, unrestricted LCSW/MSW (or equivalent) license in the state of desired employment and be able to independently practice at the master’s degree level.
  • Minimum of 1-3 years' post-master direct counseling experience with adult patients.
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.).
  • Proficiency in written and spoken English.
  • Strong computer skills, including EMR experience.

Responsibilities

  • Performs initial and follow-up evaluations, including assessments.
  • Engages the veteran, core, and expanded PACT team members in treatment planning.
  • Provides a range of treatment services, including individual therapy, typically focused on behavior activation and short-term needs for physical and behavioral health issues.
  • Provides evidence-based psychotherapies as necessary and appropriate.
  • Provides emergency care, including referrals for primary care treatment, psychiatric medication evaluation, or inpatient psychiatric unit admissions.
  • Provides crisis intervention services, addressing the cause and presenting complaint, and coordinating family conferences.
  • Acts as an advocate for the Veteran and family members/caregiver when appropriate, and educates and encourages the Veteran to self-advocate.
  • Makes rapid assessments and responds to psychiatric emergencies.
  • Provides case management to Veterans and families as needed throughout the continuum of care.
  • Addresses issues such as advance directives and organ donation.
  • Inputs all patient information into VISTA/CPRS.
  • Completes clinical reminders due at the time of each patient visit.
  • Maintains confidentiality of veteran data and information.
  • Participates in staff meetings regarding clinical, administrative, and staff development issues.
  • Provides consultation services to other staff regarding psychosocial needs of veterans and their impact on healthcare.
  • Participates in program planning and development, making recommendations for changes in policies and procedures.
  • Collaborates in performance improvement processes and complies with performance measures.
  • Participates in activities that obtain feedback from veterans and works to enhance services.
  • Complies with all Valor and VA training requirements.
  • Fulfills compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance, and other related items.
  • Remains focused on achieving excellent clinical outcomes through specified VA guidelines.
  • Provides excellent customer service to veterans, families, colleagues, and visitors.
  • Establishes and maintains a warm, welcoming, and professional atmosphere.
  • Collaborates with the interdisciplinary clinical team to enhance coordination of comprehensive patient care.
  • Collaborates with Clinic leadership to resolve veteran concerns or complaints.
  • Participates in clinic outreach events to support enrollment initiatives.
  • Supports patient enrollment and retention in appointments, follow-up visits, or nurse visits.
  • Embraces and supports new initiatives, whether clinical or operational.
  • Performs other duties as assigned.
