About The Position

Matthew 25 is seeking a Lead Housing and Case Management Specialist to provide frontline operational oversight and expert guidance to a team of Case Management Specialists and Housing Case Management Specialists. This role serves clients through the Kentucky HIV/AIDS Care Coordination Program (KHCCP) and housing programs funded by HOPWA, HOME TBRA, and the Kentucky Housing Corporation (KHC). The Lead will be the primary point of contact for day-to-day problem-solving, program guidance, and service quality, ensuring compliance with housing and HIV program standards. This role also involves supporting documentation accuracy through chart audits and care plan reviews, and promoting a Housing First approach, while maintaining a reduced caseload.

Requirements

  • Must have a Bachelor’s of Social Work or a B.S. in a related field, or have a Licensed Practical Nurse certification
  • Basic knowledge of medical case management principles.
  • Experience working in a nonprofit organization or a strong desire to work in service driven environment.

  • Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
  • Valid Driver’s License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
  • Must be able to pass a drug screen and background test.

Responsibilities

  • Provide direct case management and housing support to a reduced caseload of clients in accordance with KHCCP, HOPWA, TBRA, and HUD guidelines.
  • Serve as the go-to resource for day-to-day team support, assisting with scheduling, service delivery questions, and complex client situations.
  • Conduct regular chart audits and documentation reviews to ensure compliance, accuracy, and consistency across the team’s caseloads.
  • Support staff training, onboarding, and skill development to maintain program standards and service quality.
  • Promote a Housing First approach and ensure clients are connected to appropriate medical, housing, and social service resources.
  • Assist clients in maintaining Ryan White eligibility, accessing health insurance, and adhering to care plans and treatment.
  • Collaborate with the Program Manager on workflow coordination, performance monitoring, and continuous improvement efforts.
