About The Position

The Kidney Transplant Coordinator acts as a liaison among families, hospital staff, and patients who are preparing for or have undergone a kidney transplant. The coordinator participates in clinical management with the necessary disciplines to coordinate patient services and education across the continuum of care. This is an on-site position at Froedtert Hospital in Milwaukee, Wisconsin, with a 1.0 FTE schedule of 40 hours per week. The schedule is Monday through Friday, eight-hour days, with no weekends or holidays.

Requirements

At least 3 years of acute care or dialysis experience.
An associate's degree in nursing and at least 10 years of relevant experience, or a bachelor's degree in nursing and at least 3 years of relevant experience.
Hands-on patient experience in an intensive care setting.
Strong organizational skills and excellent communication skills.
Ability to work independently and take programmatic initiative.
Knowledge of data systems, outcomes data, and other clinical performance data.
Current Wisconsin Registered Nurse license or a multistate nursing license from a participating Nurse Licensure Compact state.

Nice To Haves

Experience relevant to nephrology or transplant subspecialties.
Bachelor's degree in nursing.
Certified Clinical Transplant Coordinator (CCTC), Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN) certification.

Responsibilities
