About The Position

Sunnyhill, Inc. is seeking a Personal Outcomes Coach to support participants in its Independent Supported Living Assistance (ISLA) Program in Jefferson County, in the Festus area. The role provides and oversees ongoing training in daily living skills and supports individuals with intellectual and developmental disabilities (IDD) in achieving goals outlined in their Individualized Support Plans (ISPs). The coach helps create a positive, encouraging, and engaging environment while ensuring guidelines are met, maintaining accurate documentation, and prioritizing participants’ safety and well-being. Previous experience in a similar role is a plus, but motivated candidates new to the field may be trained.

Responsibilities

Provide ongoing support and training to help participants achieve personal goals outlined in their Individualized Support Plans (ISPs).
Ensure applicable guidelines are met or exceeded.
Maintain detailed daily progress notes in compliance with funding guidelines.
Attend meetings, maintain documentation, and communicate with individuals, guardians, and service providers.
Assist with developing and reviewing individualized safety plans and maintaining safety supplies.
Represent Sunnyhill professionally and with kindness and a positive attitude.
