About The Position

Sunnyhill, Inc. is seeking a Personal Outcomes Coach for its Independent Supported Living Assistance (ISLA) Program in South County, St. Louis, Missouri. The role supports people with intellectual and developmental disabilities (IDD) in their homes and in the community, focusing on teaching daily living skills such as cooking, meal planning, and household organization. The goal is to help individuals maintain their homes and manage their lives successfully.

The coach provides positive support and training to help people achieve goals in their Individualized Support Plans (ISPs), ensures program guidelines are met, maintains daily progress notes, and communicates with individuals, guardians, and service providers. The role also involves participating in meetings, supporting individualized safety plans, and maintaining safety supplies. Sunnyhill describes the position as part-time, with flexible hours between 7 a.m. and 11 p.m. A sign-on bonus of $750–$1,000 is offered, depending on location.

Responsibilities

• Provide continuous positive support and training to help individuals achieve personal goals outlined in their Individualized Support Plans (ISPs).
• Ensure all guidelines are met or exceeded for people supported in the ISLA Program.
• Keep detailed daily progress notes and comply with funding guidelines.
• Attend meetings and maintain excellent documentation.
• Communicate consistently with individuals, guardians, and service providers.
• Assist with developing and reviewing individualized safety plans and maintaining safety supplies.
• Prioritize the well-being of individuals in their homes and in the community.
• Represent Sunnyhill professionally and kindly while providing quality services.
