Ensure the implementation of the WV Birth to Three (WVBTT) Individualized Family Service Plan process. This role involves contacting assigned referrals within established timelines, informing families of the WVBTT Procedural Safeguards, and establishing working relationships with Service Coordinators, practitioners, and community partners. The position also coordinates transition planning, facilitates intake and eligibility meetings, and Individualized Family Service Plan (IFSP) meetings. The role requires the ability to remain stationary up to 50% of the time and traverse various terrains to access visitation sites. Additionally, the Interim Service Coordinator will inform families and Service Coordinators of the travel reimbursement process, compile and submit reports, assist in recruiting practitioners, and participate in ongoing monitoring and continuous improvement activities. The position also involves active participation in Child Find and public awareness activities, responding to inquiries in a timely manner, and attending all required committees, conferences, meetings, and trainings relevant to the program and agency. Any additional duties as assigned will also be part of the role.
