About The Position

Reporting to the Program Manager, the Intensive Case Manager will be responsible for supporting and counselling clients experiencing mental health and addictions challenges within Region of Peel Housing Service programs. They will offer time limited Intensive Case Management and Substance Use supports as part of SHIPs collaborative team. This program will offer an integrated model of care, providing short term interventions with a goal of housing and health stabilization.

Requirements

  • Post-secondary education in Social Work, Health Services, Psychology or equivalent
  • 3+ years providing Mental Health assessments and/or case management, outreach to individuals who are homeless or at risk of homelessness
  • Demonstrated knowledge of serious mental illness, concurrent disorders, awareness and belief in the value of harm reduction principles
  • Knowledge and application of evidence-based practice for housing and mental health services & short term intensive case management (Housing First, Critical Time Intervention)
  • Excellent assessment, risk management, and crisis intervention skills
  • Expertise in collaboration with healthcare, social service, and shelter services
  • Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
  • A vehicle, valid driver’s license, and adequate insurance coverage
  • Valid Certification in First Aid & CPR
  • Training in ASIST and CPI

Responsibilities

  • Utilize standardized assessment tools to identify support needs, with a recovery and strengths-based approach
  • Establish short term support plans that align with clients expressed needs, such as enhancing life skills, addressing physical and mental health, decreasing risk in substance use, engaging in meaningful activities, and building social and community relations
  • Provide crisis intervention support and advocacy within systems of care
  • Offer service navigation to longer term mental health and substance use programming, with supported transitions in care
  • Use Housing First Principles to help clients maintain their housing and assist clients at risk of homelessness with eviction prevention. Provide information, education, and support regarding housing issues to maintain successful partnerships
  • Participate in collaboration with Region of Peel Housing Services as identified in care plans. Follow case management standards and participate in case conferencing meetings
  • Maintain appropriate record keeping/documentation, client files, statistical and outcome measurement tools, meeting agency and partner timelines
  • Maintain client privacy and confidentiality
  • Submit biweekly updates on status of caseloads and referrals
  • Administer naloxone in response to suspected opioid overdoses, and must complete appropriate training to recognize overdose symptoms, safely administer naloxone via nasal spray or injection, and follow emergency protocols including contacting emergency services. You will receive training within the first eight weeks of employment.
