About The Position

Intensive Mental Health Case Manager (Case Manager) provides high touch, in-community case management for people (clients/tenants) residing in housing or shelter programs provided by the City of Glendale Housing Division (referring partner). Care management activities are intended to improve a client’s current condition by coordinating and monitoring clinical and social needs of clients, to reduce and/or avoid the need for intensive levels of services, i.e. mental health, substance use disorder, and medical conditions. The Case Manager will be responsible for collaboratively assessing resources and needs, making referrals and linking them to services, as well as monitoring and following up to ensure that the client and family’s needs are met. Additionally, the Case Manager will be responsible for establishing and maintaining active and effective liaisons with relevant community agencies to foster linkages. Gateways Case Manager will support the Glendale housing teams and clients at every stage of the housing process—from initial engagement to housing placement and stability once housed. Services are person-centered and tailored to each client’s unique needs. This includes immediate support for basic needs, access to physical and behavioral healthcare, transportation, benefits enrollment, housing and care coordination.

Requirements

  • Ability to work and relate effectively with clients, families, provider organizations, public sector agencies and other stakeholders involved.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to remain calm, objective and dependable in situations where clients are emotional or uncooperative, and situations where there are assignments that must be met.
  • Ability to appropriately follow instructions and feedback from supervisors.
  • Functional experience with Los Angeles County mental health system, Medi-Cal system, managed care plans and related community-based organizations (CBO), etc.
  • Ability to sit for periods of 2+ hours.
  • Experience with an electronic health record system.
  • Knowledge of basic computer skills, including Microsoft office, email communication, video conferencing, and typing.
  • High School Diploma
  • Prior experience working with adults, youth and families from diverse ethnic, cultural, and socioeconomic backgrounds.
  • 3+ years working with adults experiencing severe mental illness (SMI), in a hospital, board and care, day treatment, housing or other equivalent setting
  • Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Community Care Licensing (CCL) background clearance.
  • Valid California Driver’s license.
  • TB clearance.
  • Driving record acceptable for coverage by Gateways insurance carrier.

Nice To Haves

  • B.A. or B.S. in mental health or related field, or 2 years of college plus 3 years of experience with adults with serious mental illness (SMI), in a hospital, board and care, day treatment, housing or other equivalent setting
  • 5+ years’ experience with target population providing case management and linkage services to adults, youth and families.
  • Experience participating in multidisciplinary teams.

Responsibilities

  • Conduct outreach and engagement with clients, primarily through in-person contact or their preferred communication method.
  • Work with the client and support system to conduct a thorough needs assessment and develop a care plan.
  • Organize care activities and maintain regular contact with provider organizations to coordinate all services being provided to the client (mental health, SUD, primary care, housing support, social services and other supportive services, etc).
  • Manage new referrals and facilitate ongoing client engagement and adherence to care plans.
  • Assist clients and their personal support system during transition from hospitals, other treatment settings, jail, etc.
  • Provide education and identify support needs for clients, their families, or other caregivers.
  • Offer services in person and accompany clients to appointments when necessary.
  • Share information and reports with City of Glendale teams, Gateways and other approved / authorized agencies and organizations.
  • If necessary, the case manager will provide support to the client’s family to foster reunification and/or transition to home.
  • Case manager will provide clients with education about their condition and care plan.
  • Will coordinate the client’s care across all involved providers within the client’s continuum of care.
  • Will monitor the clients’ progress and adjust the care plan as needed.
  • Performs other additional tasks as directed.
