About The Position

Under the direction of the Substance Use Health Manager, the Integrated Wellness Case Manager is responsible for coordinating comprehensive, client-centred care for adults participating in the Day/Evening Treatment Program. This role focuses on assessment, care planning, service coordination, advocacy, and ongoing monitoring of client progress related to substance use health, mental health, and social determinants of health. The Integrated Wellness Case Manager works collaboratively with clinical staff and community partners to support continuity of care, client engagement, and successful transitions to ongoing recovery and wellness supports. Services are delivered within a trauma-informed, culturally responsive, and recovery-oriented framework. To ensure the provision of treatment plans and counselling services for clients that is consistent with the philosophy, policies, and standards established by Dilico Anishinabek Family Care.

Requirements

  • Experience working with Anishinabek adults, families and communities; more specifically pertaining to mental health and substance use health.
  • A minimum of five (5) years in the adult mental health and substance use health field.
  • Experience with case management, counselling and care coordination.
  • Satisfactory Criminal Records Search.
  • Ability to work flexible hours as determined by the Substance Use Health Manager in relation to service requirements.
  • Must possess a Class “G” Driver’s License and have access to a vehicle.

Responsibilities
