About The Position

Easterseals MORC is hiring an Integrated Healthcare Specialist to support individuals served and contribute to team-based care. The role provides customer service across diverse communication channels, screens inquiries and requests, manages team charts, supports team meetings and population health activities, reviews service data, and assists with care coordination.

Requirements

High school diploma or equivalent.
Three years of responsible full-time clerical experience.
Excellent customer service skills.
Ability to follow complex oral and written instructions.

Responsibilities

Assist individuals served by addressing inquiries, resolving issues, and fulfilling requests across diverse communication channels.
Answer and triage internal and external telephone calls.
Conduct initial screening of individuals served and other patrons to determine the nature of requests or problems.
Pull, file, audit, and maintain charts for assigned teams.
Close team charts.
Co-facilitate team meeting minutes, including huddles regarding team-based care, integrated health, and other population health topics.
Review and share data reports and outcomes of team services.
Assist with coordination of care for individuals served on the team.
