The Behavioral Health Clinician (BHC) is an integral member of the primary care team, working collaboratively and consultatively to improve patients’ overall health and well-being. The clinician provides early identification, assessment, triage, and evidence-based, problem-focused interventions for behavioral, emotional, psychological, and social concerns related to physical and mental health conditions. The BHC meets with patients individually to conduct comprehensive assessments and makes recommendations to primary care providers to enhance patient care plans. When appropriate, the clinician provides follow-up treatment through skills training, coaching, and evidence-based interventions to improve health, functioning, and wellness. The role also helps patients transition smoothly to community-based behavioral health resources when they need services not available within the clinic. This position requires advanced skill in evidence-based interventions for patients with a variety of emotional and behavioral problems. The clinician works with a multidisciplinary team and must be flexible and willing to work at the fast pace of primary care.
