About The Position
Caring Hearts Homecare is looking to hire an Intake Nurse to assist in the intake process and assist in day-to-day supervision of direct patient care. Caring Hearts Homecare is a Medicare/Medicaid certified home health agency based in Wilmington MA. At Caring Hearts Homecare, we take a team approach in caring for our clients. By working with clients, families and physicians, our experienced and well trained team of clinicians are able to provide customized and individualized high quality care that meet specific needs of each of our clients.
Requirements
- Must have a current RN/LPN license in Massachusetts.
- Background in public health, home health, or hospital work.
- At least one year of clinical experience.
- Hands-on nursing experience.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Good computer skills.
Nice To Haves
- Home care experience preferred.
Responsibilities
- Assist in taking referrals and assigning patients to field staff.
- Follows up on referrals by contacting patients, physicians, family members, etc.
- Assist in handling patient care supervision and day-to-day patient care management.
- Assist in managing the HR needs of the clinical department, notifying the Branch Manager/Administrator when new hires are needed to stay proactive with the growth plan.
- Assists with new hire screenings/ interviews of home health care personnel.
- Orients professional staff to clinical policies and procedures of our Agency.
- Performs other tasks as assigned.
