About The Position

The Intake Nurse will be responsible for receiving information from referral sources, coordinating patient care, verifying and entering physician orders, identifying the level of care needed, and reviewing clinical documentation for Skilled Nursing placement. This role requires exceptional interpersonal and customer service skills, clear communication with pharmacists, and close collaboration with referring hospitals, physicians, skilled nursing facilities, patients, and families. The position also involves managing after-hours and weekend referrals through a rotating on-call schedule.

Requirements

• Current license as a licensed LPN/RN from the Utah State Board of Nursing
• 1-year Skilled nursing facility experience
• Good verbal and written communication skills
• Basic computer knowledge
• Ability to effectively problem solve
• Provide excellent customer service
• Ability to commute/relocate to Woods Cross, UT: Reliably commute or planning to relocate before starting work (Required)

Nice To Haves

• Knowledge of Medicare/Medicaid home care and hospice criteria preferred
• Knowledge of Medicare, Medicaid and 10A criteria
