About The Position

The Intake Nurse is responsible for receiving information from referral sources, coordinating care for patients, verifying and entering physician orders, identifying the level of care needed, and reviewing clinical documentation to determine Skilled Nursing placement. This role requires exceptional interpersonal and customer service skills, clear communication with pharmacists, and close collaboration with referring hospitals, physicians, skilled nursing facilities, patients, and families. The position also involves managing after-hours and weekend referrals on a rotating on-call basis. This position complies with the CMS Mandate for COVID 19 Vaccinations.

Requirements

• Current license as a licensed LPN/RN from the Utah State Board of Nursing
• 1-year Skilled nursing facility experience
• Good verbal and written communication skills
• Basic computer knowledge
• Ability to effectively problem solve
• Provide excellent customer service
• Ability to commute/relocate to Woods Cross, UT

Nice To Haves

• Knowledge of Medicare/Medicaid home care and hospice criteria
• Knowledge of Medicare, Medicaid and 10A criteria
