About The Position

Enhabit Home Health & Hospice is seeking an Intake Coordinator to coordinate patient referrals and serve as a resource for referral sources. The role collaborates with branch and sales teams to support appropriate coordination and transitions to services, and applies knowledge of regulatory guidelines and governing processes. The position requires an LPN or LVN license in the state of practice, relevant health care and home health or pediatrics experience, and dependable transportation for driving in varied weather conditions.

Requirements

• High school diploma or equivalent.
• At least two years of experience in the health care industry.
• At least one year of experience in home health or pediatrics.
• Demonstrated customer service experience.
• Demonstrated knowledge of Medicare guidelines governing home health or hospice.
• Basic demonstrated technology skills, including operating a mobile device.
• Graduate of an approved school of professional nursing.
• Licensed in the state of current practice as an LPN or LVN.
• Valid state driver's license.
• Maintain automobile liability insurance as required by law.
• Maintain dependable transportation in good working condition.
• Ability to safely drive an automobile in all types of weather conditions.
