This role provides high-quality intake support for clinical teams. PurposeCare is a leading provider of home care and home health services in the Midwest, serving over 3,000 clients monthly to ensure they remain healthy and safe at home. They focus on providing support for vulnerable individuals to live full lives within their communities.

- Ability to successfully complete a background check and professional reference check

Responsibilities
- Obtain patient information as needed
- Evaluate and assess documentation to ensure accuracy and compliance
- Ability to effectively communicate with office and clinical teams
