About The Position

The Intake Coordinator role at Patriot Homecare is a work-from-home position with initial in-office training. The business hours are Monday through Friday, 8:00 AM to 4:30 PM. Requirements

  • High School Diploma or GED equivalent.
  • Must be literate and able to maintain simple records in English.
  • Must have a criminal background check.
  • Ability to adequately hear on the telephone, with no more than an amplifier.
  • Ability to communicate both verbally and in writing in English.

Nice To Haves

  • Experience in a Home Healthcare setting.

Responsibilities

  • Answer phones and receive all intake information for paraprofessional cases.
  • Enter appropriate information into the computer system.
  • Complete intake forms and route them for admission approval.
  • Assist in scheduling appropriately skilled personnel for cases.
  • Input scheduling information into the computer.
  • Continue to assist in the coordination of scheduling.
  • Take telephone referral information.
  • Perform other duties as assigned.
