Intake Coordinator - Home Health
About The Position
The Intake Coordinator manages the full patient intake and referral process for a busy home health and hospice agency. This role includes insurance verification, authorization, data entry, and coordination of care, while serving as a key point of contact for patients, families, and referral sources. Requirements
- Healthcare experience required, preferably in home health or hospice intake/referrals
- Strong knowledge of insurance verification, authorizations, and reimbursement processes
- Excellent communication, customer service, and relationship-building skills
- Ability to work independently with strong organization and multitasking skills
Nice To Haves
- Nursing background preferred, but not required
Responsibilities
- Oversee daily intake and referral operations to ensure timely patient admissions
- Verify insurance eligibility, authorizations, and reimbursement accuracy
- Coordinate patient transitions to home health services
- Provide direct oversight of patient education and preparation for home health, plan of care initiation, and coordination of care with multiple service providers.
- Maintain strong relationships with referral sources, case managers, and healthcare partners
- Track and report referral trends and intake metrics
- Support pricing discussions and payer communication as needed
