About The Position

The Intake Coordinator manages the full patient intake and referral process for a busy home health and hospice agency. This role includes insurance verification, authorization, data entry, and coordination of care, while serving as a key point of contact for patients, families, and referral sources. Requirements

  • Healthcare experience required, preferably in home health or hospice intake/referrals
  • Strong knowledge of insurance verification, authorizations, and reimbursement processes
  • Excellent communication, customer service, and relationship-building skills
  • Ability to work independently with strong organization and multitasking skills

Nice To Haves

  • Nursing background preferred, but not required

Responsibilities

  • Oversee daily intake and referral operations to ensure timely patient admissions
  • Verify insurance eligibility, authorizations, and reimbursement accuracy
  • Coordinate patient transitions to home health services
  • Provide direct oversight of patient education and preparation for home health, plan of care initiation, and coordination of care with multiple service providers.
  • Maintain strong relationships with referral sources, case managers, and healthcare partners
  • Track and report referral trends and intake metrics
  • Support pricing discussions and payer communication as needed
