About The Position

Join Our Team as an Home Health Intake Coordinator at Alpha Home Health & Hospice! This is a full-time, on-site position at our Everett office, with working hours from Monday to Friday, 8:30 AM to 5:00 PM. Requirements

  • At least 2 years of intake experience in a medical office.

Nice To Haves

  • Experience as a home health intake coordinator highly preferred, but not required
  • Experience with Home Care Home Base highly preferred, but not required

Responsibilities

  • Perform patient intake, verify insurance, demographics, orders, and document necessary information
  • Make persuasive and empathetic outreach calls to patients to explain the value of home health services as ordered by their physician
  • Address concerns, clarify expectations, and highlight how care will help them safely recover and regain independence
  • Interface with contract, clinical, and office staff to foster collaboration and positive working relationships
  • Serve as the primary responder to all inbound calls and inquiries
  • Provide excellent customer service to patients, referral sources, and community contacts
  • Maintain proper daily workflow and ensure compliance with all regulatory requirements
  • Communicate effectively and professionally regarding coordination of care expectations
  • Educate and enforce deadlines, schedule updates, and workflow requirements
  • Coordinate initial patient assessments and follow-ups with clinical staff
  • Ensure accurate data entry and record-keeping for all patient information
  • Assist in the development and maintenance of intake procedures and protocols
