About The Position

We are seeking a dependable, detail-oriented individual to join our team as an Intake Coordinator – Home Health. This role supports both patient intake coordination and medical records administration within a fast-paced home health environment. This is a long-term position for someone interested in building a career in healthcare administration. Training will be provided. We are seeking candidates who are committed to reliability and long-term employment.

Requirements

  • High school diploma or equivalent
  • Reliable transportation and ability to travel to patient homes or field locations as needed
  • Ability to work one Saturday per month as part of a rotating schedule
  • Healthcare or administrative experience is a plus, but not required
  • Willingness to learn and grow within the role

Responsibilities

  • Coordinate patient intake for home health services, including gathering and verifying patient information
  • Prepare and maintain patient medical records and documentation
  • Communicate with patients, clinicians, and physician offices as needed
  • Assist with scheduling and coordinating home health visits for clinical staff
  • Follow up on physician orders and required documentation to ensure completeness and compliance
  • Enter and update information accurately in the electronic medical record system
  • Support general administrative and phone duties as needed
  • Maintain confidentiality and comply with HIPAA and agency policies
