Requirements

  • High School Diploma or equivalency (GED) required.
  • Some college education required.
  • Bilingual (English/Spanish/Portuguese)
  • Experience with Health IT system (organization Tools/training)
  • On-the-job training completeness/ongoing as per changes to program mandates and regulatory guidelines (CDC/ACS/NJCEED/USPTF).
  • Annual competencies completed.

Responsibilities

  • Engage new clients seeking information, coordinate intake appointments with referents and complete comprehensive intake assessments.
  • Answer incoming calls and assist with issues related to eligibility, referrals and authorizations for cancer screening.
  • Foster a supportive team environment.
  • Schedule intake visits and maintain future appointments respectful to program goals.
  • Perform file maintenance and assist care managers with client caseload.
  • Screen and orient potential members; conduct cancer screening and other service needs and assessments.
  • Direct patients to other services of Saint Michael’s Medical Center.
  • Provide assistance in tracking program enrollment and perform liaison services as needed.
  • Maintain the screening program clinic scheduling and appointment systems.
  • Coordinate timely intake process on referrals to primary and sub-specialty clinics.
