About The Position

We are seeking a dedicated Intake Case Manager with 2–3 years of experience in behavioral health settings. The ideal candidate will be skilled in completing clinical documentation, conducting thorough assessments, and coordinating care to ensure a seamless client intake process.

Requirements

  • 2–3 years of experience in case management or intake within a behavioral health or mental health setting.
  • Demonstrated proficiency in completing ASAMs, Biopsychosocial assessments, GAD-7, and PHQ-9.
  • Strong organizational, time management, and interpersonal communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • High School Diploma or Equivalent
  • CPR/FA Certification – to be obtained within first 90 days
  • Fingerprint Clearance Card – to be obtained within first 90 days
  • Handle with Care – to be obtained within first 90 days
  • Successful clearance of pre-employment criminal/county/state background check
  • Successful clearance of pre-employment drug and alcohol testing
  • Successful clearance of TB PPD Test upon hire and annual screening for TB symptoms.

Nice To Haves

  • Familiarity with electronic health record (EHR) systems is preferred.

Responsibilities

  • Conduct comprehensive intake assessments with new clients, ensuring all documentation is completed accurately and timely.
  • Complete clinical documentation including: ASAM (American Society of Addiction Medicine) assessments, Biopsychosocial assessments, GAD-7 (Generalized Anxiety Disorder) and PHQ-9 (Patient Health Questionnaire) screenings.
  • Collaborate with clinical and administrative staff to ensure clients are appropriately placed into services.
  • Maintain up-to-date client records in accordance with organizational policies and regulatory standards.
  • Support clients with resource referrals, follow-ups, and appointment coordination as needed.
  • Adhere to all HIPAA and confidentiality regulations.
  • Participate in case reviews, supervision, and staff meetings.
