About The Position

The Intake Assessment Coordinator is responsible for managing and coordinating screening assessments of individuals seeking inpatient treatment at Gentle Heart Recovery-managed facilities. This role involves collaborating with marketing and outreach teams to manage intake calls, conduct pre-screening assessments, and determine the most appropriate level of care for patients. The coordinator also educates referral partners, assists with patient transportation arrangements, verifies insurance, and ensures all necessary paperwork is completed and updated in various systems.

Requirements

  • High School Diploma or Three (3) years of experience as an Intake Coordinator (or equivalent position) working in the substance use treatment field.
  • Knowledge and competence of local communities.
  • Outstanding interpersonal skills, with an emphasis on the capability to successfully build professional relationships.
  • Excellent written and verbal communications.
  • Strong time management capability.
  • Ability to multitask and prioritize effectively.
  • Ability to utilize the Microsoft Office Suite and other necessary computer and technology platforms.
  • Adept at strategizing and executing business.

Responsibilities

  • Manage and coordinate screening assessments of individuals seeking inpatient treatment.
  • Answer intake calls and voicemail messages.
  • Conduct pre-screening assessments of potential patients to determine the most appropriate level of care (detox or rehabilitation).
  • Report information on expected patients for admissions, updating Airtable and referral paperwork.
  • Educate referral partners and community resources on services, facilities, availability, admissions, and treatments.
  • Follow up with prospective patients approved for treatment to secure transportation.
  • Input information related to transportation services into Microsoft Teams and Airtable.
  • Coordinate with potential referrals to develop criteria and documentation for appropriate level of care.
  • Verify insurances of prospective patients, utilizing the ePACES NY Portal for NY Medicaid insured individuals.
  • Submit requests for Verification of Benefits (VOB) for third-party or commercial insurance.
  • Inform patients of their responsibility for insurance deductibles and co-payments.
  • Initiate case conferences with the Director of Admissions or Director of Clinical Reimbursement for admissions approval when needed.
  • Follow up to ensure the completion of referral paperwork.
  • Communicate and inform the Admissions Department on the status of all expected patients.
  • Post patient screening assessments to applicable facilities.
  • Report significant patient medical or clinical concerns to the Intake Manager, Medical Director, and/or Executive Director.
  • Coordinate all patient transportation requests with the Transportation Coordinator and post updates in Microsoft Teams.
  • Review the intake grid on Airtable for daily expected patients and reservations.
  • Inform patients about facility policies and procedures, including setting, admissions process, and treatment programs.
  • Adhere to transportation safety guidelines under all governing agencies.
  • Perform other duties as assigned.
