The Advanced Practice Provider (APP) in Inpatient Palliative Care is responsible for performing complete histories and physicals, ordering and collecting data, and serving as the first responder for patient status changes. The role involves assessing risks associated with acute and complex chronically ill patients, collaborating with the interdisciplinary health care team to make diagnoses, managing diagnostic tests, and formulating differential diagnoses. The APP will formulate and implement plans of care, prescribing diagnostic strategies and therapeutic interventions, and may perform advanced procedures. Documentation includes daily progress notes, transfer and discharge summaries, and other relevant medical records, ensuring timeliness and compliance with acute care standards. The position also requires ongoing communication and collaboration with the health care team, patients, families, and staff to ensure continuity of care. Professional practice behaviors include preceptor/mentoring, education, engagement in research, participation in grand rounds and specialty meetings, maintaining CEUs, and professional organization membership.
