About The Position

The Chaplain Inpatient Palliative Care provides pastoral care, counsel, emotional support, and spiritual/religious rituals to patients, families, and staff. This role involves conducting spiritual assessments, collaborating with healthcare teams to identify holistic patient care needs, and facilitating progress towards resolution or adjustment to illness. The chaplain also plans and leads chapel and memorial services, provides sacraments, and makes appropriate referrals when requests are outside their scope. Additionally, this position involves facilitating small groups, providing in-service education, and representing the department on committees. A commitment to ongoing learning and professional development is essential.

Requirements

  • Master's Degree of divinity or arts in theology with concentration in pastoral care
  • 2 years of relevant experience in pastoral care
  • CRT-Chaplain and Fellow - UNKNOWN
  • LIC-Driver's License - STATE_MI State of Michigan

Responsibilities

  • Conducts and records spiritual assessments of patients.
  • Provides pastoral care, counsel, emotional support, and spiritual/religious rituals to patients, families, and staff.
  • Identifies available resources and assists with referrals.
  • Documents interventions provided.
  • Collaborates with departmental colleagues on shared responsibilities, referrals, charting, logging, flexibility, patient hand-offs, follow-ups, pager coverage, and mutual support.
  • Collaborates with the interdisciplinary health care team in identifying holistic approaches to patient care needs and facilitating progress.
  • Plans and leads chapel and memorial services.
  • Provides sacraments as needed.
  • Offers appropriate referrals when a request is outside the chaplain’s ability to provide.
  • Facilitates small groups within the organization or community.
  • Provides in-service education for relevant hospital units, students, personnel, and community leaders.
  • Represents the department on assigned committees and task-forces.
  • Demonstrates a commitment to ongoing learning and professional development.
  • Takes initiative in identifying and accessing relevant opportunities for learning and growth.
