About The Position

Sentara Virginia Beach General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN) for the Emergency Dept (ED)! This position is eligible for up to a $20,000 sign-on bonus for qualified candidates. This is a Full-Time, permanent position (40 hours) with standard working hours of a 4x10 shift: 9:00 AM to 7:30 PM EST, Sunday through Wednesday or Wednesday through Saturday. This is an onsite position at Sentara Virginia Beach General Hospital, 1060 First Colonial Road, Virginia Beach, VA 23454.

Requirements

• Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
• BLS within 90 days of hire.
• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
• 1 year of nursing experience REQUIRED

Nice To Haves

• Bachelor's Degree in Nursing preferred
• 1 year acute care case management preferred
• Emergency Dept (ED) OR Intensive Care Unit (ICU) experience preferred

Responsibilities

• Responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.
• Directly support those in hospital patients identified in need of comprehensive case management.
• Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions.
• Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver.
• Functions in one of the following practice settings: Hospitals Only
