About The Position

Sentara CarePlex Hospital and Sentara Louise Obici Hospital are seeking a dedicated Inpatient Case Management Supervisor, RN to join their collaborative team. This full-time, day-shift opportunity is ideal for an experienced and motivated nurse leader ready to elevate their career by managing dedicated inpatient case management teams across two exceptional Sentara facilities—CarePlex Hospital in Hampton, VA, and Obici Hospital in Suffolk, VA. This role is for individuals who thrive in fast-paced, acute care environments and are passionate about patient advocacy, team development, and operational excellence. Requirements

  • Bachelor’s degree in Nursing (BSN)
  • Current Registered Nurse (RN) license—either Virginia or Compact/Multi-State
  • Basic Life Support (BLS) certification from the American Heart Association (required within 90 days of hire)
  • Specialty certification required within one year of eligibility (acceptable certifications include ACM, CCM, CCCTM, CMAC, or CMGT-BC)
  • At least 3 years of case management experience

Responsibilities

  • Supervise the day-to-day workflow and performance of the case management team at both hospital locations
  • Ensure effective discharge planning and seamless coordination of inpatient care
  • Support length-of-stay management, patient flow processes, and related performance improvement initiatives
  • Provide coaching, mentoring, and constructive feedback to staff to foster professional growth
  • Address employee relations matters in partnership with leadership and HR
  • Maintain compliance with departmental standards, hospital policies, and regulatory requirements
