About The Position
This position is responsible for executing tactical, analytical, and strategic work to ensure the model of care team meets clinical and performance goals and has the greatest clinical impact. The role will significantly contribute to shaping the future of integrated care at DaVita. It provides strategic direction and tactical oversight for model of care initiatives, coordinates program teams and initiative plans, develops strong relationships with internal and external partners, and monitors and manages model of care program performance.
