About The Position
An Independent Optometrist leases space inside a Walmart store and takes a leading role in patient care related to eye and vision care, as well as general health and wellbeing. The independent lease holder has autonomy to establish practice days and operating hours, control the patient schedule, set fees, and retain 100% of examination and professional fees. For fair market value, the lease holder may opt into equipment and instrumentation, administrative support, and/or opt-tech support services where permitted by state law. This is an independent leasing opportunity, not a position of employment with Walmart Inc. or any Walmart-owned entity. The stated primary location is 950 W Votaw St, Portland, IN 47371-1254, United States of America.
Requirements
- Doctor of Optometry (OD) from an accredited optometry school.
- Valid state optometry license.
- Ophthalmologist MD or DO degree from an accredited school.
- Valid state ophthalmology license.
- General liability certificate.
- Professional liability insurance certificate.
Responsibilities
- Provide patient care related to eye and vision care and general health and wellbeing.
- Establish days of practice and hours of operation.
- Control the patient schedule and set the fee schedule.
- Retain 100% of examination and professional fees.
- Optionally obtain equipment, instrumentation, administrative support, and/or opt-tech support services for fair market value, where permitted by state law.
