Requirements

• Bachelor of Social Work or related field
• Experience working with families in social services context
• Valid driver's license with properly registered and insured vehicle
• Proficient in Microsoft Office
• Time management and multi-tasking skills
• Writing and communication skills

• Ability to generate well-written, lengthy reports
• Capacity to build a rapport with teens and young adults
• Ability to work as a team to assist youth with exiting foster care safely and appropriately

Responsibilities

• Facilitate the movement of children through the permanency process
• Facilitate resources for the youth and help prepare the youth for independent living
• Help the youth identify resources and supports
• Meet with the youth to help them problem solve and address and resolve independent living concerns
• Provide casemanagement services
