Under the direction of the Family Preservation Program Manager, the In Home Outreach Clinician will provide one-hour, in-home counseling and clinical case management sessions to families in and around Service Area 7 once per week, at minimum. This role involves conducting thorough intake and assessments, unannounced visits, child follow-up sessions, and arranging/attending Multidisciplinary Case Planning Committee Meetings. The clinician will also provide resources and referrals, collaborate with the Department of Children and Family Services, and maintain accurate documentation. Additionally, the position requires attending weekly supervision and program meetings, and conducting one-time Family Preservation Assessment Services.
