About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The position focuses on providing flexible, comprehensive, and personalized health evaluations. The visits, lasting approximately 45 minutes, allow for one-on-one connection and addressing health questions. The evaluation includes a review of medications and medical history, a physical assessment, and potentially diagnostic tests if ordered by the health plan. Clinicians will use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter existing treatment plans.

Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
