About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, or airborne allergens.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification (required).

Responsibilities

Conduct In-Home Health Evaluations.
Engage with people in the comfort of their own homes.

Provide flexible, comprehensive, and personalized health evaluations.

Perform physical evaluations.
Conduct additional diagnostic tests if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.

Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
Be flexible to travel locally and within licensed states, as needed.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
