In-Home Health Clinician - Nurse Practitioner or Physician Assistant (Full Time)- Grand Junction, CO
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Grand Junction, Colorado area. Clinicians visit people in their homes to build trusted relationships, help identify gaps in care, and connect members with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, when ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medication, ordering lab tests, or changing a person’s existing treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The position requires local and in-state travel as needed, and coverage of the Grand Junction MSA, including Mesa, Delta, Montrose, Ouray, Pitkin, Gunnison, and Chaffee counties.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to safely and comfortably perform duties in members’ homes, which may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional Diagnostic & Preventive Services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Build trusted relationships with members and help connect them to appropriate care.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Provide Signify Health services as indicated.
