About The Position

Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians visit health plan members in their homes, building trusted relationships and providing flexible, comprehensive, and personalized health evaluations. Visits last approximately 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen. The role primarily takes place in members’ private residences, which may contain household pets, secondhand smoke, or other potential airborne allergens or irritants. Local and in-state travel and virtual visits may be needed. The role covers Grand Junction and the surrounding area. Part-time employees may qualify for select benefits, malpractice insurance coverage during Signify Health-related activities, supplies, and other perks.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to cover Grand Junction and the surrounding area.
• Ability to comfortably and safely perform duties in private residences that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer members’ health questions and help connect them with appropriate care.
• Provide additional diagnostic and preventive services when indicated or ordered, such as diabetic eye exams and spirometry.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services indicated for the role.
• Work as a clinician colleague in accordance with the requirements of the specified role.
