About The Position

Nursing and Therapy Services of Colorado (NTSOC) is seeking an experienced and compassionate IHSS RN Coordinator to support high-quality in-home care for pediatric and adult clients. The role coordinates IHSS/Agency-based services, supports individualized care planning, ensures compliance, and serves as a clinical resource for home-based care teams. The coordinator works with leadership and interdisciplinary staff to assess client needs, support service authorization and ongoing eligibility, and provide training. This is a primarily office-based role with regular travel to client homes, suited to an RN with home health experience, strong clinical judgment, and a commitment to supporting individuals with disabilities. The position is full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. It may involve client home visits, occupational hazards such as communicable diseases, harmful chemicals, and disoriented or combative clients, and working under stress and adapting to changes in activity.

Requirements

Graduate of an accredited nursing program.
Minimum of 3 years of RN clinical experience.
Strong communication and computer skills.
Colorado RN license in good standing with the Department of Regulatory Agencies.
BLS certification.
Current car insurance and a valid driver's license.
Ability to meet the position's physical and mental demands, including regularly using hands to handle objects, tools, or controls; communicating effectively; occasionally standing, walking, sitting, and reaching; regularly driving to client homes; and coping with work stress and changes in activity.

Nice To Haves

Bachelor of Science in Nursing (BSN).
Minimum of 2 years of nursing experience in home health or a similar healthcare setting.
IHSS experience.

Responsibilities

Coordinate and implement IHSS/Agency-based services, including health maintenance activities, personal care, and homemaker services for pediatric and adult clients.
Coordinate and complete client admissions, ensuring required assessments and documentation meet regulatory and organizational standards.
Review client needs comprehensively to support eligibility determinations and service authorization.
Collaborate with case management on service authorizations and completion of required documentation.
Participate in developing, implementing, and regularly reviewing individualized care plans, updating them to reflect current client needs.
Oversee delivery of IHSS/Agency-based services to ensure compliance with regulatory and organizational standards.
Train attendants and complete skills-validation checkoffs at initial onboarding and annually.
Work with clients or authorized representatives to troubleshoot staff concerns and concerns related to care plans.
Track and conduct periodic onsite supervision of in-home attendants, complying with state and federal regulations and completing required EHR documentation.
Answer questions from IHSS/CFC attendants and apply working knowledge of skilled versus unskilled care.
Cross-train and support Clinical Services with skilled-care needs and coverage as operational needs arise.
Potentially manage a partial caseload of clients receiving clinical nursing services as needs arise.
Participate in a scheduled on-call hotline rotation, providing timely clinical support, triage, and guidance to clients, families, and staff.
Perform other duties as assigned.
