RN Care Coordinator
About The Position
The Registered Nurse (RN) Care Coordinator uses nursing judgment and critical-thinking skills in a collaborative care-coordination process to advocate for consumers and improve health outcomes. The RN reviews records to determine appropriate care, assists with transitions of care, ensures the appropriate quality and quantity of services, collaborates with the treatment team, and provides the core team with educational materials and training. The RN assesses patient data to identify potential gaps in care, tracks and trends laboratory work and hospital and emergency department utilization, and uses a disease-management focus to guide the care-coordination team.
Requirements
- Associate degree in nursing.
- Two years of clinical experience.
- Licensed Registered Nurse in the State of Texas.
- Valid Texas driver's license with an acceptable driving record.
- Provide the required license, if applicable, and official documentation of educational attainment, such as a transcript, degree, or certificate.
Responsibilities
- Review records to determine appropriate care.

- Ensure appropriate quality and quantity of services are provided.
- Collaborate with the treatment team.
- Provide the core team with educational materials and training.
- Assess patient data to identify potential gaps in care.
- Track and trend laboratory work and hospital and emergency department utilization.
- Guide the care-coordination team using a disease-management focus.
- Advocate for consumers and facilitate improved health outcomes through care coordination.
