This position is primarily responsible for identifying, coordinating, and performing intake and continuity of care services for persons with Intellectual and Developmental Disabilities. This position will require you to interact with individuals, families, community agencies, and service providers. Work will include educating others about IDD services provided through the Center, services and supports provided through HHSC, coordinating and facilitating eligibility determinations, managing the IDD General Revenue and Statewide interest Lists, and assisting in coordination of placement into Medicaid Waiver programs, Intermediate Care Facilities, and State Supported Living Center Facilities. Duties will also include support tasks for the IDD Unit, including data entry into CARE, TMHP, CSIL, and Netsmart systems, checking and facilitating Medicaid eligibility, attending monthly CRCG meetings that occur in the rural counties that the Center covers, and general filing and clerical support duties. Must be self-directed and articulate, demonstrative of good judgment and comfortable working with minimal oversight.
