About The Position

This position serves as the lead Disease Intervention Specialist with the regional hub, primarily covering Gadsden County. The incumbent provides advanced epidemiological and investigative support for Gadsden County and assists with investigations involving both Gadsden and Leon counties. The role involves conducting comprehensive communicable disease investigations, interviewing individuals infected with or at risk for communicable diseases, and carrying out field investigations to locate clients, deliver Field Directed Therapy, and facilitate access to counseling, medical evaluation, and treatment services. This position ensures complete linkage to care for newly diagnosed HIV-positive individuals in Gadsden County, with all activities documented in the HIV/AIDS Linkage modules. The incumbent also performs ongoing case management, including documentation, prioritization, clustering, visual case analysis, and applying epidemiological techniques. Additionally, the role includes providing Rapid HIV and Syphilis testing, delivering results within required timeframes, conducting confidential pre- and post-test counseling, and collaborating with healthcare providers to verify diagnoses and treatments. Provider outreach is conducted to encourage reporting, share disease trends, and promote adherence to guidelines. All investigative reports and documentation must comply with CHD and Department of Health policies. The incumbent also serves as a Field Investigation Trainer, providing instruction and mentoring to new and existing Disease Intervention Specialists (DIS) across a 14-county service area, focusing on client engagement, investigation procedures, linkage-to-care processes, and data collection. Field assessments are conducted quarterly to evaluate staff performance and training needs. As a certified 500/501 Trainer, the incumbent delivers training on 500/501 course content to new DIS staff and community partners, ensuring material reflects current program guidelines and best practices. The role also involves assisting with annual updates for the 500/501 training. The incumbent actively participates in in-service training and staff development, completes required Department of Health trainings, and stays current on communicable disease trends, program protocols, and policy updates. Additional responsibilities may be assigned as needed.

Responsibilities

  • Conduct comprehensive communicable disease investigations, including interviewing individuals infected with or at risk for communicable diseases to obtain partner, cluster, and suspect information.
  • Carry out field investigations to locate clients, deliver Field Directed Therapy, and facilitate access to counseling, medical evaluation, and treatment services.
  • Ensure all newly diagnosed HIV-positive individuals in Gadsden County are followed through complete linkage to care, with all activities documented in the HIV/AIDS Linkage modules.
  • Perform ongoing case management duties, including case documentation, work prioritization, clustering, visual case analysis, and the application of epidemiological techniques consistent with program standards.
  • Provide Rapid HIV and Syphilis testing, deliver test results within required timeframes, and conduct confidential pre- and post-test counseling.
  • Collaborate with physicians, clinics, laboratories, and other healthcare providers to verify diagnoses, treatments, and laboratory findings.
  • Conduct provider outreach to encourage reporting, share disease trends, and promote adherence to current diagnostic and treatment guidelines.
  • Complete all investigative reports, case documentation, and laboratory-related forms in accordance with CHD and Department of Health policies and procedures.
  • Provide instruction, mentoring, and technical support to new Disease Intervention Specialists (DIS) and to DIS staff requiring additional guidance throughout the 14-county service area.
  • Build staff competency in interviewing and engaging with clients in the field, field investigation procedures, linkage-to-care processes, and accurate data collection.
  • Conduct field assessments quarterly or more often as needed to evaluate staff performance, training needs, and adherence to program standards.
  • Serve as an instructional resource for new DIS staff and community partners who require 500/501 course content.
  • Deliver formal training sessions as needed, ensuring all material reflects current program guidelines, legal requirements, and best practices for communicable disease intervention.
  • Assist in preparing and presenting the annual updates for 500/501 as needed.
  • Actively participate in in-service training programs, staff development activities, and other professional learning opportunities relevant to program performance.
  • Complete all required Department of Health trainings within established timeframes.
  • Maintain current knowledge of emerging communicable disease trends, program protocols, investigative methods, and policy updates.
  • Perform other duties as assigned.
