This full-time position requires a Bachelor's Degree in a Human Services field and two years of experience working with children and families, or a Master's Degree in a Human Services field and one year of experience working with children and families. The role involves planning, coordinating, and documenting services for clients and their families to ensure their educational, medical, emotional, and social needs are met. Responsibilities include providing and coordinating ancillary and social services, maintaining records, preparing reports, communicating with clients and stakeholders, responding to crises, creating and maintaining Individual Service Plans (ISPs) and Individual Crisis Management Plans (ICMPs), conducting service plan meetings, acting as a liaison between clients and various stakeholders, developing discharge plans, and coordinating visits.
