Temp- Housing Supports Program Case Manager
About The Position
The Housing Supports Program (HSP) provides supportive housing services to help individuals secure safe, stable housing and maintain a stable living environment. Case managers provide support in participants’ homes and the broader community, delivering outreach and case management to help participants access, obtain, and effectively use resources suited to their individual needs. The program also supports participants in developing and carrying out independent living skills. This is a temporary, full-time position based in Bayville/Ocean County, working 35 hours per week. The role is part of HSP/Adult Residential Services and pays $21.00 per hour. The position is not benefits-eligible.
Requirements
- High school diploma with life experience.
Nice To Haves
- Experience working with health insurance companies is a plus.
Responsibilities
- Provide supervision and support to clients.
- Help clients identify housing-related needs and set goals to address them.
- Assist clients with transportation, recreational activities, and medical or psychiatric appointments.
- Support clients in learning and implementing daily living skills to enhance independence.
- Deliver case management services to help clients sustain their benefits and housing.
- Consult with clinical staff regarding client concerns and requirements.
- Conduct therapeutic rehabilitative skill development.
- Contribute to intake assessments, needs assessments, and housing support plans.
- Review support plans collaboratively with clients.
