SLCS is currently seeking a full-time Housing Stabilization Worker for the HART Hub. The HART Hub (Homeless and Addiction Recovery Treatment Hub) supports individuals who are experiencing or at risk of experiencing homelessness and who face challenges related to substance use and/or mental health. As part of this collaborative and client-centered team, you will work alongside healthcare providers, housing specialists, and community partners to reduce barriers, connect individuals with meaningful resources, and support access to safe, appropriate, and permanent housing. In this role, you will manage a caseload and work directly with clients to establish and achieve personal goals—particularly those related to maintaining safe, stable housing as a foundation for long-term well-being. You will act as a strong advocate for clients, coordinating care and collaborating with a wide range of community partners, including emergency shelters, healthcare providers, income support services, and the justice system. Your work will be grounded in a trauma-informed, culturally safe, and compassionate approach that reflects the HART Hub’s commitment to low-barrier, integrated care.
