About The Position

At Your Support Services Network (YSSN), we believe in making a difference. As a non-profit charitable organization, we support individuals with mental health challenges, developmental disabilities, and complex needs—helping them build meaningful connections and live their best lives. The Housing Stability Outreach Worker provides front-line homelessness prevention and shelter diversion supports for people in York Region who are experiencing homelessness or are at imminent risk of losing housing (within 30 days). The role delivers mobile, community-based outreach; completes housing stabilization and diversion planning; coordinates short-term emergency assistance; and connects people to crisis services, mental health supports, developmental services, income supports, and other community resources to support sustainable housing outcomes.

Requirements

  • Bachelor’s degree in Social Work or related discipline (i.e. Health Sciences)
  • Minimum three years related experience
  • Providing community-based outreach, housing stabilization, homelessness prevention/diversion, or intensive case management
  • Demonstrated knowledge of mental health, developmental disabilities, and the impacts of trauma, poverty, and housing instability
  • Strong skills in advocacy, service navigation, and coordinated care planning with multiple partners.
  • Excellent interpersonal communication skills, including de-escalation and conflict resolution.
  • Ability to maintain accurate documentation and work with data/outcomes tracking requirements.
  • Working within a coordinated access/homelessness-serving system and with prevention & shelter diversion funding/program models.
  • Supporting hospital discharge planning or transitions from public institutions.
  • Knowledge of harm reduction practice and community-based substance use supports.
  • Familiarity with landlord-tenant systems and local housing resources.
  • A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
  • Must be able to provide a police clearance that meets agency standards.
  • Must submit proof of a primary series of a COVID-19 vaccine authorized by the World Health Organization.
