Riverside Community Care is seeking a Housing Coordinator to join the Adult Community Clinical Services (ACCS) Integrated Teams. These teams provide clinical interventions and peer and family support to help individuals manage their symptoms, improve their functioning, and live successfully in the community. The Housing Coordinator will be a key member of the ACCS team, with the primary responsibility of supporting individuals in obtaining and maintaining stable, secure housing. This role involves educating staff and clients about tenancy issues, housing rights, and available resources, assisting clients with subsidy applications, helping them find housing that meets their needs, guiding them in developing skills for community housing maintenance, and serving as a liaison with landlords, housing authorities, and DMH Area housing staff to facilitate communication and provide advocacy.
