About The Position

The DHS Housing Case Manager is responsible for providing comprehensive case management services to individuals and families experiencing homelessness or housing instability and are residing in a shelter setting. The Housing Case Manager works collaboratively with clients, service providers, landlords, and community agencies to develop individualized housing plans, connect clients to resources, promote housing stability, and support long-term self-sufficiency. The Housing Case Manager will work closely with the social service team.

Requirements

  • Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field.
  • Minimum of two years’ experience providing case management or housing services.
  • Knowledge of DHS housing programs, public assistance benefits, and community resources.
  • Strong organizational, communication, and documentation skills.
  • Ability to manage multiple cases while meeting deadlines.

Nice To Haves

  • Experience working with individuals experiencing homelessness, mental illness, substance use disorders, or other vulnerable populations.
