Reporting to the Manager of Housing and Client Care, the incumbent will be responsible for providing client needs identification through ongoing assessment and client-directed care planning. This role involves implementing client-centered plans of care focused on strengths, wellness, and empowerment to support recovery and goal achievement. The Case Manager will work cooperatively with clients and their Clinical Case Managers to address housing needs, and will develop, seek out, and recommend potential market-based landlords. Key functions include implementing client-centered approaches to promote housing stability, providing crisis prevention and intervention planning to prevent relapse and/or hospitalization, offering short, non-intrusive monitoring support, and operating under the Housing First model.
