About The Position

Primary Health Care (PHC) is seeking a dedicated Housing Case Manager to join The Project team. PHC was founded in 1981 with a mission to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay. PHC offers a spectrum of medical and dental services, and its Homeless Support Services acts as the entry point for serving people experiencing homelessness in Polk County. Enabling services such as benefits enrollment, case management, transportation, translation, and patient education are available. PHC has locations in Ames, Des Moines, and Marshalltown.

Requirements

  • Bachelor’s degree in human services field or equivalent combination of education experience.
  • Minimum of 2 years of experience working with clients who need assistance relating to homelessness, housing, and/or advocacy for underserved populations.
  • Excellent customer service skills.
  • Effective written and verbal communication skills.
  • Proficiency using Microsoft Office applications including Excel and other database applications.
  • Problem solving and critical thinking skills.
  • Ability to multi-task.
  • Demonstrates professionalism with ability to maintain composure in all situations and show empathy.
  • Current Mandatory Reporter Training: Child and Dependent Adult Abuse recognized by the Iowa Department of Public Health; Active Chauffer’s license from Iowa Department of Transportation. Both must be obtained within introductory period if not current.

Responsibilities

  • Assisting clients with finding and maintaining appropriate housing options.
  • Developing and maintaining open communication with community partners, landlords, neighborhood groups, and other organizations to build capacity for affordable housing options, coordinate services and provide education about PHC services.
  • Managing a housing assistance budget according to grant guidelines.
  • Monitoring and documenting client contacts, housing goals, progress, and significant occurrences, according to grant guidelines and PHC policies and procedures.
  • Collecting, recording, and reporting data related to housing clients.
  • Facilitating coordination with other PHC services, including medical, dental, mental health and substance abuse, as well as referrals for services with external agencies, including emergency and crisis services.
  • Educating PHC staff about program focus, landlord/tenant issues, housing barriers, and related services.
  • Performing other duties as assigned.
