About The Position

The primary role of the RRH Case Manager is to provide rapid rehousing advocacy and case management services to survivors of domestic violence and/or human trafficking. Case management utilizes a Housing First philosophy, includes trauma-informed, resiliency-focused and survivor-centered approaches to stabilize housing, rebuild self-esteem, develop essential life skills, and establish financial independence. This is achieved by supporting a client's goals to increase income and self-sufficiency. Additionally, the advocate will provide survivors and their children with counseling, risk assessment, safety planning, goal setting, and referrals to other support services.

Requirements

  • Two years' experience in client advocacy, counseling, supportive housing or similar setting
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Ability to accept responsibility and follow through with assignments
  • Experience or demonstrated interest in domestic violence and residential programs
  • Willingness to learn how to navigate multiple systems including housing, health, employment, substance use disorder treatment and government benefits
  • Willingness to fully support the mission of the organization
  • Local travel necessary
  • Reliable transportation required

Responsibilities

  • Provide services that are client-centered, trauma-informed and resiliency-focused
  • Ensure client confidentiality (as outlined in CGS 52-146K and PCC's confidentiality policy)
  • Ensure compliance with funding requirements and contracts and PCC's policies and procedures
  • Work collaboratively with PCC staff, CT Coalition Against Domestic Violence (CCADV) staff, and other community service providers to address client needs
  • Complete intakes, assessments, and screenings to determine housing eligibility based on HUD and CT Department of Housing (DOH) guidelines
  • Complete survivor-centered, strengths-based housing plans that identify barriers to housing and set a path to housing stability; reviewed and updated monthly
  • Support survivors in designing and implementing safety plans and housing risk assessments
  • Provide home visits or community visits, safety permitting, at a minimum of once per month, with each enrolled survivor
  • Provide case management services focused on increasing income, accessing long-term housing subsidies, establishing and/or expanding family and community-based supports, and assisting in increasing access to community services
  • Develop and/or maintain a relationship within the homeless service system
  • Assist in program development and quality assurance activities
  • Assist survivors with locating available housing, the leasing process, landlord mediation, and tenant/landlord rights and responsibilities
  • Submit requests for rental payments for enrolled survivors
  • Assist with protective/restraining orders and other court matters
  • Arrange for emergency medical services or law enforcement services as needed
  • Provide/arrange transportation for clients as needed
  • Other duties as assigned
  • Maintain accurate and up to date files and records to include required documentation and confidential case management notes
  • Complete and submit: Timesheets and monthly Personnel Activity Reports (PARs), Annual reports as needed, RRH HMIS quarterly reports
  • Document all client and program data in EmpowerDB within 48 hours of contact
  • Acquire and maintain certification as a Domestic Violence Counselor
  • Attend local CAN meetings
  • Participate in the annual Point-in-Time count
  • Attend CCADV, HUD-related and DOH trainings and workshops related to housing
  • Assist in additional agency operations as requested
  • Actively participate in staff trainings and meetings
  • Assist in fundraising and community awareness activities
  • Record and compile statistics as required
